Cullybackey Medical Centre and Galgorm Surgery - There is a Temporary Patient List Freeze You will be aware of recently received news of the terminations from the above named GP Surgeries in the Ballymena area.  We have been advised to implement a temporary freeze on the movement of patients from these two practices within the same catchment during the termination notice period, i.e., Cullybackey Medical Centre 30th November 2023 Galgorm Surgery 31st August 2023.

This means that any patient who is currently registered with Cullybackey Medical Practice and Galgorm Surgery should remain registered and should not move to another practice within the same catchment area. 

New Patient Registration

New patients wishing to register (must live within an 8 mile radius, practice postcode is BT44 9LF) can do so by completing a pre-registration form (this can be collected at the practice or download from below) and returning it to the practice with your medical card.  If you no longer have your medical card then please complete the HS200 form (below) and attach it with your registration form. A receptionist will then contact you to advise that you have been accepted by the practice and arrange a suitable time for an initial appointment with a doctor. 

Patients no longer register with a designated general practitioner but within the practice. 

Patients are encouraged to register at the practice nearest to their address.

Carers of Patients 

If you are a carer or have a carer please inform reception and they will give you a form to complete for our records.

Change of Personal Details 

It is important to keep us informed of any changes to your personal details such as, your name, address or telephone number to keep our records up to date (your details can be updated by using the link on the homepage of this website). We also inform Business Services Organisation for their records and any correspondence they need to send you.
